JOB SUMMARY
The Diabetes Program Manager promotes individual and population health by managing and facilitating the National Diabetes Prevention Program (NDPP)and other lifestyle change initiatives in various settings served by the Health Federation of Philadelphia. The Program Manager will provide oversight of the establishment of the NDPP and other behavioral change programs associated with CDC funded Diabetes Prevention and Management project (Cooperative Agreement 2320, Component B) and the Integrated Peer Models for Health (IPMH) programs. This includes developing and implementing health access initiatives and education that center on fitness, nutrition, and health hygiene. These initiatives have a focus on improving health and expanding access to high-quality care for underserved and vulnerable individuals and families.
The Program Manager will work alongside and supervise the Lead Community Health Worker as they collaborate with partner organizations to recruit NDPP participants, orient potential participants to the program, and enroll them in groups. In addition, the Program Manager will supervise contractual Lifestyle Coaches hired to support program implementation of the NDPP at the partner sites. The Program Manager will also collaborate with the Project Director of the CDC Diabetes project and the IPMH program in the implementation of other CDC behavioral change programs and related initiatives.
For this role, we will seek to hire an individual with leadership and communication skills experienced in working with patients with Diabetes and/or pre-diabetes. Training and experience as an NDPP Lifestyle Coach and experience with Federally Qualified Health Centers is highly desirable.
JOB SPECIFICATIONS
Responsibilities/Duties
- Manage, recruit for, and facilitate the National Diabetes Prevention Program (DPP);
- Function as DPP lifestyle coach at health center and/or community settings.
- Support HFP Lead Community Health Worker in program implementation and execution
- Direct NDPP development – ensure fidelity of curriculum, identify training opportunities, work with partner organizations, help to organize data management plan
- Work with partner organizations and the HFP team to recruit patients and manage program materials
- Organize training and ongoing support for Lifestyle Coaches and support the start-up of Diabetes Prevention Program groups at community locations.
- Gather, maintain, and submit data for program monitoring, evaluation, improvement, and funding efforts;
- Attend and participate in city-wide meetings and workgroups such as the Philadelphia Diabetes Prevention Collaborative & ensure relevant information is shared back
- Develop innovative and creative ways to facilitate NDPP groups virtually and in person
- Maintain and serve as Health Coach to DPP lifestyle program Alumni Group
- Maintain knowledge and expertise in information relative to assigned programs;
- Support the implementation of one or more Family Healthy Weight Programs at partner organizations.
- Provide periodic health education sessions to community members in North Philadelphia
- Support relevant program implementation, training activities, and technology literacy initiatives with partner organizations, and community members of different experience, comfort, and literacy levels;
- Supervise Lead CHW and contract Lifestyle Coaches working within partner organizations.
- Participate in coalition-building and advocacy related to the role of CHWs in Diabetes Prevention and Management
Education
- Bachelor’s degree in Social Work, Public Health, Public Administration, Nutrition, Education, or a similar field from an accredited college or university preferred. May be substituted by relevant work experience.
Skills/Experience
- Experience working as a CDC Diabetes Prevention Program lifestyle coach with adults from urban communities that have a disproportionate burden of poverty and chronic disease.
- A passion for teaching people about healthy lifestyles and motivating participants to learn.
- Experience in group facilitation
- Knowledge, and experience in health education, nutrition and/or exercise program implementation.
- Skilled in verbal and written communication, well organized, computer literate, and capable of managing a variety of activities.
- Able to work a flexible schedule, including leading/supporting classes on nights and weekends.
- Demonstrates leadership qualities including time management, listening skills, problem-solving, critical thinking and decision-making, priority setting, work delegation, and work organization.
- Able to develop positive, longitudinal relationships and set appropriate boundaries with patients/families.
- Experience using tools like Microsoft Office and Google Suite for Business. The ability to quickly master new technology tools is essential.
- Ability to travel locally in Philadelphia
- Good knowledge of Philadelphia neighborhoods and the resources of these communities
- Spanish language proficiency is a plus
Work Environment: Standard office setting with extended periods at work station and periodic use of office equipment.
Position Type and Work Schedule: Full-time position, typical hours are Monday through Friday 8:30 am to 5:00 pm. Flex office schedule options are available with supervisor approval.
Travel: This position requires travel to sites in the Philadelphia area
Physical Demands: Ability to transport materials and routinely move equipment or supplies weighing up to 30 lbs.
