HFP received news on Friday, July 15 that our expanded Health Center Controlled Network has been funded by the Health Resources and Services Administration (HRSA) for the three year cycle of 2022-2025. This cooperative agreement funded at $1,040,817 for the 2022-2023 grant year supports HFP’s work with 33 community health centers in Pennsylvania in support of their capacity to leverage health information technology and data to deliver high-quality, culturally competent, equitable, and comprehensive primary health care, with a specific focus on improvements in clinical quality, patient-centered care, and provider and staff well-being.
This is the fourth round of HCCN funding that HFP has received since 2012, and reflects the rapid growth of our Network, which started at ten health centers. Over 75% of health center patients in PA are served by participants in HFP's HCCN. The HCCN team is excited to continue and expand this important work around the state.
