JOB SUMMARY
The Medical Case Manager will provide practical, educational, and emotional assistance to Philadelphia Health center clients affected by HIV/AIDS. The Medical Case Manager will support the client’s ability to manage activities of daily living to increase and/or maintain quality of life and obtain and maintain medical stability and medication adherence.
Medical Case Management services are provided under the direction of, or by referral from, a clinical provider and include client centered services that implement the clinical treatment plan, including linking clients with health care, psychosocial, and other services. The coordination and follow-up of medical treatments are key components of medical case management. These services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the client’s and other key family members’ needs and personal support systems. Medical case management includes the provision of treatment adherence counseling to ensure readiness for, and adherence to complex HIV/AIDS treatments.
The Medical Case Manager will have internal contacts with the entire administrative staff and external contact with clients, community resources, and the community. This position has access to sensitive information and is expected to handle such information with integrity and professionalism. This position has regular contact with members of the community and is expected to represent the City of Philadelphia in a professional manner. The Medical Case Manager will be based in the Philadelphia Health Centers.
The Medical Case Manager will report directly to an overseeing supervisor and participate in department activities as necessary, as well as any trainings or direction set forth by the health center.
JOB SPECIFICATIONS
Essential Responsibilities/Duties
- Conduct initial pre-screening of patients to determine eligibility for services and appropriateness of case management services.
- Develop a comprehensive care plan jointly with the patient that includes short and long term goals focused on attaining, maintaining, and achieving positive health outcomes.
- Establish and implement a medical treatment plan by providing counseling on medications, appointments and other treatment adherence issues.
- Work with patients and team to identify barriers to care and implement interventions with Health System Navigators to remove barriers to care.
- At least twice annually evaluate and update the client care plan based upon changes to health status and other factors detailed in the reassessment.
- Conduct risk reduction counseling and partner counseling and provide referral services with all patients to prevent HIV transmission.
- Refer and link clients to appropriate services within the system of care that promote positive health outcomes, treatment adherence, and greater self-sufficiency.
- Monitor the client’s follow-through with these services.
- Evaluate effectiveness of services based upon client outcomes in the scope of work.
- Other tasks as deemed necessary.
Non-Essential Responsibilities/Duties
- Attend relevant workshops or join professional groups as necessary to maintain professional knowledge and licensure.
- Attend relevant staff meetings to promote communication and execution of goals.
- Complete special projects specific to the function of the department or as needed for the department as directed by Supervisor.
- Other duties as assigned within the scope of position expectations.
Education
Minimum Experience: 2+ years experience providing direct services to a diverse population in a human service, health care, community, or educational setting. Experience with maternal, pediatric, and family health issues and insurance/medical assistance procedures with economically disadvantaged are preferred. Bi-lingual in English/Spanish or English/French is preferred.
Minimum Education: Bachelor’s degree preferred.

- Work will be significantly in the community, primarily in the field- you will be required to travel throughout Philadelphia County.
